Enroll In The Program

Go to  to enroll in the Program.

You must attest to the five principles or you can not join the Program and proceed to the rest of the enrollment form.

Click Next:  Make CERTAIN that you have clicked Paul Cholak in the drop down box as the person who referred you.


The enrollment site is designed so that you can complete the enrollment without any assistance. 

There are two questions that you may not know the answer to.  One asks if you are in a direct primary care program.  In almost all instances the answer will be NO.  The exception is if you are a member of a concierge medical program like MDVIP.  If you live in Colorado or Arizona, other types of direct primary care (DCP) programs may be available.  If you have any question about  whether of nor you are in such a program, please call us at 877-734-3884.  

The other question asks if you want to include the Liberty Rx program in your enrollment.  This program costs an extra $27.50 per month, and we have found that less that .2% of our members benefit from joining Liberty Rx.  The concercierge/member adviser will be able to review your medications, if any, and determine if programs like World Meds or good.rx apply to you and if they will save you money over being in the Liberty Rx program.  If you find that you'd benefit from the Liberty Rx program at a later time, you'll be permitted to enroll then..

If you enroll by the 20th of the preceding month, enrollment will become effective the first of the following month.  Enrollment is YEAR-ROUND and is NOT restricted to the Obamacare Annual Enrollment period, which runs from November 1 to December 15 in states that participate in and extends longer in some states (e.g. Colorado) that run their own individual state Exchanges (enrollment systems).

After you complete the enrollment process you'll be receiving emails from both Sedera Health and MPowering Benefits.  These emails contain instructions for downloading tools like the Sedera mobile application, completing the required health risk assessment, signing up for the Teladoc program, and registering for the personalized  one-on-one15 minute introduction to the Program (the call explains all the elements of the Program, how to maximize the benefits of your membership, the procedures to be followed, provide answers to any questions that you may have and completing any necessary requirements, e.g. as the health risk assessment.  Please note that some or all of these emails may go into your spam folder, so it is very important for you to check your email and to make sure that you read and act in accordance with the instructions contained in these emails.  (We've found that some members either unintentionally delete these emails or can't find them, and these members are missing out on very important information and procedures they need to understand and follow in order to be highly functioning members of the program and to meet requirements like completing the health risk assessment.)
